primary purpose of this position is to plan, organize and direct
the Minimum Data Set (MDS) process in accordance with current
applicable federal, state and facility standards, guidelines and
regulations. - This position is responsible for appropriate
reimbursement through accurate completion of the MDS tool. -
-Essential Functions, Key Duties, & Responsibilities
- Conduct and coordinate the development and accurate completion
of the resident assessment (MDS, CAAS and Care Plans), in
accordance with current rules, regulations and guidelines governing
the facility.
- Complete timely submissions of the MDS and monitor all related
reports including but not limited to Validation reports, Case mix
reports, 5-Star Report, Quality Measures, HMX/Vision reports,
PointRight, etc. Communicate appropriate information to the
Interdisciplinary Team/Management Team. - Participate in QAA
process as appropriate.
- Ensure resident trackers, discharges and MDSs are opened/closed
appropriately, ARDs and Completion dates are set and entered on the
Resident Care Conference Schedule, resident interviews are
scheduled/completed, and resident/responsible party notices for
Resident Care Conferences are sent in a timely manner.
- Responsible for communicating all necessary MDS details to the
Interdisciplinary Team in a timely manner to assure accurate
complete of MDS process.
- Verify accurate completion of resident face sheets including
ranking diagnoses and coding on admission and throughout MDS
process.
- Ensure accurate and timely completion of assessment and forms
related to the MDS including but limited to Braden, Skin Grids,
Admission, Falls, Restraints, Side rails, AIMS, Bowel and Bladder,
etc.
- Participate in daily Interdisciplinary Team standup and
Utilization Review meetings. Provide input and assist in completion
of Medicare Certifications/Re-certifications, managed care updates
and pre-certifications, NOMNCs/ABNs, DENCS/KePro appeals and any
other requests for information for reimbursement. - Work with the
nursing team to assure thorough and appropriate documentation that
meets all regulatory guidelines.
- Oversee and assist in the ongoing Triple check process assuring
completion per facility policy and guidelines.
- Maintain and periodically update written policies and
procedures that govern the development, use and implementation of
the MDS process.
- Attend and participate in continuing education, workshops,
seminars, etc., to keep abreast of current changes in the
healthcare field and the MDS process.
- Will participate in case management of any insurance, Medicare
or Medicaid residents.Position RequirementsEducation: Associate's
or Bachelor's degree in NursingLicenses/Certifications: active LPN
or RN license that permits practice in the state the associate is
hired to work in Experience: 1-2 years of experiene in MDSSkills &
Abilities:
