• Certification Details
• BLS, MMR, Varicella, Hepatitis B, Tdap, and Influenza vaccinations or titers.
• Fit Test, PPD, IGRA, and physical health statements.
• All certifications and health histories are subject to verification.

  • Will need to work 4 late shifts during the 13-week contract.
  • May have to work weekend or holiday shifts.
  • May need to take call.

      • Additional Details
      • American Heart Association BLS certification required for clinical positions.
      • Primary Source Verification of professional license and/or certification required within 30 days of start and rerun every 2 years with RN license renewal & upon expiration.
      • Temporary Licenses NOT accepted - must have a valid permanent license.
      • Proof of COVID-19 vaccine series completion preferred, not required.
      • Positive MMR and Varicella titers required within the last 15 years with booster doses under specific conditions.
      • Fit Test completed within 1 year of start updated annually.
      • Proof of full Hepatitis B series or declination required.
      • If positive PPD, CXR within 5 years and current Questionnaire required.
      • Proof of Tdap vaccination within past 10 years required, declinations not accepted.
      • 2-Step PPD or IGRA required if not done within past year (annual requirement).
      • Influenza Vaccine required from Oct-March unless exemption approved.
      • Completion of UHS modules required prior to clearance.
